By offering a full range of programs and services to seniors right in their homes, Mercy Community HomeCare is able to help many seniors achieve their goal of staying at home for as long as possible.
Here are some of the programs and services available to seniors through Mercy Community HomeCare. Should a client's needs change over time, our Mercy Continuum of Care is also available to meet all medical and spiritual needs.
Skilled Nursing Services
Under the direction of a physician, and with the participation of both clients and their families, we develop nursing care plans that promote, restore and maintain health. Examples include:
- Wound care and dressing changes
- Medication administration and management
- Case management for complex clients
- Referrals to appropriate community resources and other disciplines
Medical Social Work Services
When emotional, psychological and behavioral problems begin to affect quality of life, we can help. We work with clients and their families to resolve the obstacles that impede our clients' ability to cope with illness or achieve recovery.
Wound and Skin Care Management
With the direction of a client's physician, we provide a comprehensive management program for the following:
- Stage II, III and IV wounds
- Surgical wounds, vascular ulcers and pressure sores
- New or problematic ostomies
- Diabetic skin problems
Homemaker and Home Health Aide Support Services
Our homemakers and home health aides assist clients with personal care and daily living activities, such as dressing and bathing. In doing so, our goal is to enhance a client's independence and quality of life. Our aides also offer socialization and companionship to those who have become isolated, as well as relief to their caregivers. The care we provide includes:
- Assistance with personal care, hygiene and dressing
- Positioning in bed and assistance with walking
- Carrying out a home exercise program developed by a therapist
- Meal preparation for regular or special diets
- Light housekeeping
Rehabilitation Program
Our goal in delivering rehabilitation services is to optimize or restore an individual's functional abilities and personal independence. Our rehabilitation team works together to deliver physical, occupational and speech therapy, as well as other necessary rehabilitation services.
