Sub-acute care at Saint Mary Home is designed for adults of all ages who have been hospitalized and need further care between their hospital stay and their return to the community. A Greater Hartford resource for decades, the Saint Mary Home sub-acute care unit and our highly skilled team provide patients with transitional care through a combination of skilled services, including extensive medical and physical rehabilitation that serve a variety of medical conditions.

For patients requiring ongoing complex treatments, we offer a milieu of services and support to them and their families. These care services are provided in a distinct unit that offers extra amenities to accommodate our clients' shorter stays. Private rooms are also available on a first-come, first-serve basis.
