Our Performance Improvement Outcome Measurement Program helps us
deliver optimal care and continually improve the quality of life of all
residents. The program also helps us assist each client/resident to
achieve the maximum level of independence and health.
The Performance Improvement Outcome Measurement Program is designed to
objectively monitor and evaluate quality of care, pursue opportunities
to improve the delivery of care and resolve identified problems. This
program is based on achieving established center/agency goals,
exceeding customer expectations and meeting all State and Federal
regulation and accreditation standards, including CARF/CCAC (the
Commission on the Accreditation of Rehabilitation Facilities and the
Continuing Care Accreditation Commission).
The program focuses on investigating and solving problems that
have the greatest impact on resident care while using resources as
efficiently as possible. It has three levels of concentration:
Clinical, Resident-Focused and Organization-Focus.
